1. YOU Submit Order Form

   To begin the process, you will need to fill out a simple order form online, which can be found here. The form is easy to complete, with only four short sections that need to be filled out.

   • Patient Information: This section is the basic background and contact information for the person whose forms are being ordered.
   • Your Information: This section is only necessary to fill out if you are ordering forms for someone else, such as a family member or client.
   • Doctor’s Information: In this section, you will need to provide the doctor’s name, phone number and office location so that we know who to contact and where to retrieve the documents.
   • Order Information: Here you will specify how many years back you want the records to go, as well as your delivery method and if you require a rush order or specialization.

   There are four – possibly only three – sections that need to filled out. Seems simple, right? That’s because it is!

2. WE Request the Records

   After we receive your order form, we will send the signed authorization, as well as the order specifications, to your doctor. This will make them aware that they need to find your medical records and have them ready.

3. WE Follow Up

   The next step is to follow up with your doctor in three to four days to make sure that they have located your records, and that they are available for us to scan.

4. WE Scan the Records

   Once we have received confirmation from your doctor that the medical records have been located and are available, we will send one of our certified field agents out to scan them.

5. WE Conduct Quality Control

   After your medical records have been scanned using our advanced technology, they will be inspected by our quality control team. We will ensure that your records are easy to read, and just as clear as the originals.

6. WE Send the Records

   The final step is to send the scanned documents to you. The way your medical records are returned to you will depend entirely on the delivery method that you selected when placing the order. They can be downloaded online via our secure platform, or shipped to you.
