Oh, the joys of turning 18 in the United States! This milestone gives you more rights and freedoms from voting in general and primary elections, to playing the lottery, to getting married and buying a house, to voluntarily joining the military, to working full-time hours at a job. Of course, this also means that you’ll have more responsibilities and duties: taking part in jury duty, mandatorily registering with the Selective Service if you’re a male, and potentially finding your own place and living on your own if your parents prefer that you move out by this age.
Turning 18 means a lot of things, but something you might not know is that it can also make you eligible to get a copy of your medical records. Having a copy of your medical records can be very beneficial in that it can give you insight to your state of health and medical history, as well as come in handy if you ever land in the emergency room or doctor’s office for an unexpected appointment.
It’s true that you must be 18 years of age or older to get a copy of your medical records without permission from a parent. However, apart from the age requirement, there are other requirements that you must meet to be approved to receive a copy of your medical records in the state of Florida.
Once at the age of 18, you must also not have a disability that prevents you from having access to your records. Certain disabilities or previous diagnoses such as drug addiction may prevent you from getting access to your medical records or part of your medical records for your safety and mental health.
Last but not least, you must be able to provide full and accurate information when filling out the medical record copy request form. This information includes, but is not excluded to, some of your personal information (e.g., your full name, birthdate, etc.), your doctor’s information (e.g., their full name, their office address, etc.), order information, and billing information. Missing or inaccurate information may cause your request for your medical record copy to be denied.
If you’re 18 or older and meet, or can meet, the latter requirements, it’s time to get your application sent in for request. Our form can be accessed and filled out online from this link.
